Osteoporosis and Dental Health

What do we know about osteoporosis? We may know that this disease makes the bones more brittle and vulnerable. We may know that osteoporosis is the cause of many a broken hip or curved spine as we age. We may even know that, for a number of reasons, women are far more likely to develop this disease. What we may not be aware of is the impact osteoporosis can have on our dental health.

“Osteoporosis” means “porous bones.” It is a disease that makes the bones more likely to fracture or break, as the body’s careful balance of absorbing old bone tissue and replacing it with new healthy bone tissue is disrupted. We lose bone tissue faster than we can create new, dense bone tissue. Why is this important for our dental health? Because the fitness of our teeth depends on the fitness of the bones surrounding and securing them in our jaws.

How does osteoporosis affect dental health?

  • Osteoporosis reduces density in the bones and bone tissue that hold our teeth in place. Studies have shown that women with osteoporosis have significantly more tooth loss than women without the disease.
  • Periodontitis, or gum disease, can also cause deterioration in the bone surrounding the teeth. This is a time to be proactive with gum health to avoid infections and further bone loss.
  • Denture wearers may find that their dentures no longer fit properly due to changes in bone structure. Bone loss needs to be addressed promptly to avoid having to replace dentures.
  • Rarely, bone-strengthening medications for osteoporosis can lead to serious jaw problems after dental procedures that involve the jawbone (such as extractions). Always tell us any medications you are taking before we schedule any dental treatment.

Unfortunately, osteoporosis often has no symptoms at all—until the first bone fracture. Checking our bone density is important as we age, and one way of discovering changes in bone density is through your regular dental checkups at our Hellertown, PA office. We can pinpoint changes in your X-rays through the years and will recommend that you see your physician if there is any indication of bone loss. Outside activities? You slather your kids with sunscreen. Biking? You don’t let your child leave the house without a helmet. Youth sports? You provide mouthguards and padding and headgear and all the other tools designed to keep your child safe. Protecting your child is a fundamental part of parenthood, and you take your job seriously.

Protecting your child’s dental health is fundamental, too! Tooth decay is the most common chronic disease in children, and children’s premolars and molars are far more vulnerable to decay than any of their other teeth. You can help protect your child’s molars and premolars with a simple and effective treatment—dental sealants.

  • How Do Sealants Work?

The bacteria in plaque use food particles to create acids. These acids erode the minerals in tooth enamel, creating weak spots that become cavities over time. Molars and premolars are especially vulnerable to cavities because of their uneven chewing surfaces. The dips and grooves on top of the teeth—technically known as “pits and fissures”—collect food particles and bacteria, and can be difficult for children to clean completely when brushing. That’s why cavities are so common in newly erupted molars.

Dental sealants protect your child’s molars and premolars by creating a barrier that covers and smooths out the chewing surface of the tooth, preventing bacteria and food particles from getting stuck inside those uneven pits and fissures.

  • What Kind of Sealants Are Available?

The two most common dental sealants are composite resin coatings and glass ionomer sealants.

With resin sealants, after each tooth is cleaned and dried, an etching solution will be brushed onto the top surface of the molar. This etching slightly roughens the surface so that the sealant will stick to the tooth more effectively. A thin coat of the sealant is then painted on and hardened under a special curing light.

Glass ionomer sealants use a flexible paste that bonds to the tooth and hardens within minutes. While they generally don’t last as long as resin sealants, they are designed to absorb and release fluoride for extra cavity-fighting protection.

  • Do Sealants Work?

They certainly do! According to the Centers for Disease Control, sealants can prevent 80% of the cavities in molars and premolars, which is where 90% of children’s cavities appear. Dental sealants can last from three to five years, or even longer. Dr. Jason P. Tosto will check the condition of any sealants at each dental exam, and let you know if it’s time for a reapplication.

  • When’s the Best Time to Get Sealants?

Tooth enamel gets harder as we get older, so children’s newly erupted teeth are more at risk for cavities. First adult molars usually arrive when a child is six to seven years old, and second adult molars come in around the age of 12. The first and second premolars can erupt between the ages of ten to 12. As soon as the first permanent molars begin to erupt, it’s a good time to talk to Dr. Jason P. Tosto about the best time to apply sealants.

And what about baby teeth? Even though baby teeth are meant to be replaced, they shouldn’t be lost to tooth decay before they are ready to fall out. Primary teeth help young children learn to speak and eat properly and hold the place for adult teeth so these permanent teeth come in where they should. Baby teeth have thinner enamel, and so cavities can progress more quickly. Dr. Jason P. Tosto can let you know if sealant protection for your child’s baby molars is indicated.

What are the five things I should do in between visits?

When it comes to keeping your smile looking its best, good oral hygiene is a must! Good oral health habits should start early and continue throughout your lifetime. Here, Dr. Jason P. Tosto and our team at Tosto Family Dentistry suggest five habits worth adopting that will help keep your teeth healthy:

  • Brush and floss regularly. Brush gently at least twice a day, paying special attention to the gum line to rid your mouth of food and bacteria that may lurk in between your teeth. Floss at least once a day. Replace your toothbrush every three to four months or sooner if the bristles are frayed.
  • Make regular visits to see Dr. Jason P. Tosto. Regular checkups (twice yearly) will help diagnose any dental problems early on when they can be more easily treated.
  • Stop smoking. Did you know smokers are four times as likely as nonsmokers to develop periodontal (gum) disease? Tobacco, whether in the form of cigarettes, pipes, cigars, or chewable tobacco, increases oral and throat cancer risks, and raises the risk for candidiasis, an oral fungal infection. Smokeless tobacco contains sugar, which furthers your risk for cavities.
  • Limit your alcohol intake. Heavy drinking dramatically increases the risk of developing mouth and throat cancers.
  • Eat healthy. Avoid snacking on foods that contain high levels of sugar or starch. We encourage you to eat plenty of fruits and vegetables, which are known to help stimulate the flow of saliva to re-mineralize tooth surfaces and neutralize cavity-causing bacteria.

The Hazards of Smokeless Tobacco

Many smokers believe that chewing tobacco is a safer alternative to smoking cigarettes. This simply isn't the case! In fact, smokeless tobacco can cause serious health concerns.

Smokeless tobacco comes in many forms and goes by many names: dip, snuff, snus, or simply chewing tobacco. Use of these products usually involves sucking or chewing on shredded or loose tobacco leaves, sometimes flavored, for a prolonged period. There are even products that emulate a dissolvable candy-like consistency which are made of compressed tobacco powder.

What are risks and smokeless tobacco?

Whichever form a tobacco product takes, the dangers of using or consuming them is very real. According to a 2007 study by the World Health Organization's International Agency for Research on Cancer, there are upwards of 28 cancer-causing chemicals in smokeless tobacco that are known to cause cancer. And these products are habit-forming just like any other tobacco product that contains nicotine. Using them will increase your risk for many serious diseases including but not limited to: cancer (especially oral and esophageal), gum and heart disease, cavities, and pre-cancerous mouth lesions.

At the end of the day, long-term use of smokeless tobacco can cause serious health issues. These products really take a toll on both your oral and overall health. They put a strain on your immune system and make it less capable of warding off infection and disease.

Dr. Jason P. Tosto and our team strongly advise you to stop using smokeless tobacco—or any kind of tobacco product—and not to pick up the habit if you aren't. There is no safe level of tobacco use, smokeless or otherwise.

The National Cancer Institute offers information, support (local and online), and tools to help smokers and smokeless tobacco users quit. They offer live online chat with cessation counselors Monday through Friday and even have a smartphone application available to help people who are serious about quitting.
